Tilt-shift cinematography tilts the lens relative to the sensor so the plane of focus rotates, and may also shift the lens to slide perspective. A narrow band or shaped region of sharpness, often from a high viewpoint, can make a real-scale scene resemble a small model.

Narrative function
Tilt is not a blur sticker. The Scheimpflug relation puts a slice of the world in focus along a plane that is no longer parallel to the sensor: a street of rowers, a crowd, a tabletop of people who suddenly read as toys.
Shift is a different job: it slides the image to keep verticals parallel or to look around a corner without rotating the camera. It is the wrong tool when you only wanted a face isolated from a wall.
That is Shallow Focus on a normal plane. It is also the wrong tool if the miniature effect has no high viewpoint and no consistent focus band.

How it works
Tilt the lens so the focus plane cuts through the subject as a thin band or a wedge. A high angle helps the diorama reading because we look down on models.
